    I'm developing a course on the topic of organizational safety. Related topics include attention, mindfulness, mindful organizing, and sensemaking. Currently, I'm seeking exercises, activities, and cases pertinent to safety and its attendant topics. I'd very much appreciate any materials or suggestions you can provide.

    Please also include ergonomics . There are many practical as well as interesting exercises available  in this area.

    I recommend you look at a book by Steven Spear "The High Velocity Edge", contrasts safety issues at NASA versus the safety record at Alcoa.

    Eric - this is getting to be a very big topic.  Depending on what you mean by a "case" or "exercise", there are lots of things out there.  Books have been written about the major disasters (Anthony Hopkins writes one every year, or every disaster, whichever comes first) and some of the major reports have influenced entire industries (e.g., Piper Alpha).  I have written papers that describe cases or examples of incident investigations (Carroll, J. S., Rudolph, J. W. & Hatakenaka, S. (2002). Lessons learned from non-medical industries: Root cause analysis as culture change at a chemical plant. Quality and Safety in Health Care, 11, 266-9) and large-scale change efforts (Carroll & Hatekenaka Driving organizational change in the midst of crisis.  Sloan Management Review, 2001, 42, 70-7).  I have attached some teaching materials on the Millstone "case" as an example.  - John

    I've used several safety-related simulations and eLearning programs in my training and development courses that might be of interest to you.  I have students work through the training programs and evaluate the quality of the content, methods, and design, what trainees are likely to learn, etc., which works well in a T&D course.  Below are links to a few programs I've used.

     

    When do you take refuge? Decision Making During Mine Emergency Escapes. (NIOSH has additional publically available safety training materials.)

    Professor Dov Zohar (The Technion, Israel) has been conducting research on organizational safety climate since 1980 and he and others have repeatedly shown how perceptions of safety climate link with incidents and accidents. He has an informative review of this literature available in the Oxford Handbook of Organizational Climate and Culture edited by me and Karen Barbera (2014).
