    There is also the
    Leader Behavior Description Questionnaire (LBDQ) available at http://fisher.osu.edu/research/lbdq/

    These two assessments may be close to what you are looking for... best of luck!

    Robert Liden created an instrument to measure four variables of LMX. The instrument is named the LMX-MDM. In my email conversations with Dr. Liden, he stated that it is in the public domain.

    Hi:

    With respect to leadership, I don't think it is really meaningful to distinguish between skills and behaviors (if the latter are appropriate for the context, as validated by predicting a theoretically useful outcomes, they are skills). Appropriate behaviors for leaders (i.e., skilled performance) are what knowledge is to mechanics (i.e., skilled performance); at the end of the day, the leader must be able to lead and the mechanic must be able to fix the car. Thus, skilled performance is a dependent variable not an independent variable. Defining a construct by its outcome (or using a loaded term like "skilled") leads to a tautology, see:

    MacKenzie, S. B. (2003). The dangers of poor construct conceptualization. Journal of the Academy of Marketing Science, 31, 323-326.

    So, a leader will be skilled to the extent that they have the ability to learn (i.e., intelligence) and sufficient experiential knowledge to have learned; they must also demonstrate suitable behavioral repertoires, many of which are caused by natural behavioral tendencies due to personality. That these traits are to a large extent heritable makes them mostly exogenous; thus, their predictive effect on outcomes can be estimated. So, I would focus on traits and behaviors only.
