Jacqueline, in addition to other comments regarding your questionnaire items, the EEOC now recommends that "Hispanic" be separated from "races," since "Hispanic" refers to an ethnic category that can include "races." For example, a number of Hispanics are also Black. I'm not sure the appropriate way to word this given you are surveying an international population, but it may be something you should consider.
